Takotsubo cardiomyopathy

たこつぼ心筋症は特に女性に多い疾患だが、エストロゲン濃度の低下が原因の一つである可能性が示された。ラットを使った動物実験で明らかになったもので、3月17日のECS-JCSジョイントシンポジウム「Pathophysiology and Clinical Aspect of Takotsubo Disease」で、和歌山県立医大の上山敬司氏が発表した。

 上山氏らは、たこつぼ心筋症が閉経後の女性にも目立つことからホルモンの関与を疑い、その解明を目指し実験を重ねてきた。たこつぼ心筋症が身体的あるいは精神的なストレスで誘発されることが少なくないことを踏まえ、実験では、 Kvetnanskyらが報告したラットのストレスモデルを用いた。上山氏らは、このモデルで、たこつぼ心筋症で見られる心電図や左室造影の所見を再現することができ、またアドレナリン受容体拮抗剤により、こうした所見を抑制することができることを確認した。

 その上で実験では、エストロゲンを補充することにより、ストレスで誘発される心電図や左室造影の所見がどのような影響をうけるのかを検討した。その結果、エストロゲン補充で、ストレスで誘発された左心室機能障害(特に頻脈)を抑制し、中央交感神経細胞におけるニューロンの活性を抑えることもできた。また、副腎や心臓における細胞活性を抑え、心臓保護効果があるHSP70やANP濃度を上昇させる効果も確認された。

 これらの結果を踏まえ上山氏は、更年期の後に続くエストロゲン濃度の低下は、神経系への間接作用あるいは心臓への直接作用によって、たこつぼ心筋症の発症に関与していることを示唆すると結論付けた。

(注)たこつぼ心筋症は、胸痛や心不全などの急性心筋梗塞症様の臨床所見が見られるが、心臓カテーテル検査では冠動脈に狭窄や閉塞などの異常所見がない。左室造影で心尖部に一過性の高度な収縮不全を認める症例があり、その収縮異常の形態的特徴から「たこつぼ型心筋症」と呼ばれている。1990年に佐藤光氏らが世界で初めて報告した。最近では、新潟大医学部の調査により、2004年の新潟県中越地震の直後、被災地の女性に、たこつぼ心筋症が多発していたことが明らかになった。

Clinical manifestations and diagnosis of arrhythmogenic right ventricular dysplasia

INTRODUCTION — Arrhythmogenic right ventricular dysplasia (ARVD), also called arrhythmogenic right ventricular cardiomyopathy (ARVC), is an underrecognized clinical entity characterized by ventricular arrhythmias and a specific ventricular pathology [1-3]. ARVD is characterized macroscopically by a fatty appearance of the RV free wall. The fibrofatty replacement of the RV myocardium initially produces typical regional wall motion abnormalities that later become global, producing RV dilation. The tissue replacement can also involve area of the left ventricle (LV) with relative sparing of the septum [4].

The incidence of ARVD is unknown, but the prevalence in the general population is estimated to be approximately 1:1000 [1]. ARVD is an important cause of sudden cardiac death (SCD) in young adults in northern Italy [5,6], accounting for approximately 11 percent of cases overall and 22 percent in athletes [6].

In contrast, ARVD has been rarely diagnosed in the United States. This may reflect a difference in genetic prevalence, but is more likely due to under recognition of disease. In a series of 100 ARVD patients from a single referral center in the United States, clinical profiles and event rates were similar to those reported in Europe [7].

The clinical manifestations, diagnostic criteria, and evaluation of ARVD will be reviewed here. The genetics, pathogenesis, treatment, and prognosis of ARVD are discussed separately. (See "Genetics and pathogenesis of arrhythmogenic right ventricular dysplasia" and see "Treatment and prognosis of arrhythmogenic right ventricular dysplasia").

CLINICAL PRESENTATION — Clinical perspectives of ARVD primarily arise from experience with patients who present with arrhythmia of right ventricular (RV) origin and/or sudden death. Presentation is most common between the ages of 10 and 50, with a mean age at diagnosis of approximately 30 years [7-9]. The disease is virtually never diagnosed in infancy and uncommonly before the age of 10.

ANATOMY AND HISTOLOGY — The anatomic and histologic findings associated with ARVD fall into several categories. Structurally, the hallmark findings are RV dilation and myocardial thinning although some patients also have LV involvement. Histologically, fibrofatty infiltration of the myocardium is the most common finding. In addition, electron microscopy has shown abnormalities of desmosomes in the RV myocardium, which is consistent with mutations in genes encoding desmosomal proteins being responsible for ARVD in most patients. CPB(cardiopulmonary bypass)

1) 組み立て
  人工肺、貯血槽等の構成部品を回路チューブで接続し、血液が循環する人工心肺回路とする。
 回路は機械側回路と術野側回路に分かれており、術野側回路は完全な清潔操作で組み立てる。
 構成部品とチューブ、あるいはチューブとチューブとの接続の多くはコネクターで行う。また、
 採血や気泡抜き、圧ラインなどは連結チューブやルアコネクター、三方活栓などを用いる。

 2) 洗浄と充填
  人工心肺構成部品はエチレンオキサイドガス(EOG)滅菌されており、残留ガスは溶血など
 を起こすため洗浄を行う。洗浄液には5%グルコース液が多く用いられる。
  充填量は使用回路と人工肺によって、使用する薬剤量の多くは患者体重により決定する。充
 填液はいったん貯血槽に蓄えてから満たす。この時送血回路の気泡抜きを行う。

 3) ヘパリンの投与と吸引の開始
  人工心肺回路内部での血液凝固を抑制するため、カニュレーション前にヘパリンが投与され
 る。投与後薬効が十分であるか確認するためACT値を測定する。ACT値が400秒を超えない場合
 は追加投与の必要がある。

 4) カニューレの挿入
  カニューレ挿入時には直ちに体外循環が開始できる状態でなければならない。送血カニュー
 レは上行大動脈または大腿動脈に挿入される。カニューレと人工心肺を接続後、気泡を除去し、
 送血テストを行う。送血回路の遮断鉗子が外された時、送血圧が患者血圧により上昇するのを
 確認する。さらに若干の充填液を実際に送り、異常な回路内圧の上昇がない事も確認する。
  脱血カニューレは、右房内または上大静脈と下大静脈に挿入される。

 5) 体外循環の開始
  人工肺へ酸素ガスの吹送を始め、送血ポンプの流量を徐々に上げるとともに脱血を開始する。
 体外循環と同時に血液は充填液によって希釈され、末梢血管抵抗は急激に減少する。これに伴
 い急激な血圧の低下が生ずる事が多く、この場合還流量を増し対処する。この時点では生体の
 心臓も活動しているため、循環血液の一部が人工心肺により循環されている。この状態を部分
 体外循環と呼ぶ。

 6) 冷却
  冷温水槽により人工心肺に付属する熱交換器に冷水を送り込み、血液の冷却を開始する。体
 温の低下とともに酸素消費量は低下するため、冷却を行う。

 7) 全体外循環
  冷却により心筋温度が低下すると心室細動を誘発し、生体心臓のポンプ機能は失われる。こ
 の時点から人工心肺だけで全身還流を行っていることになり、これを全体外循環(完全体外循
 環)という。心停止液を冠動脈に注入し停止させる場合もある。

 8) 大動脈遮断
  心臓内部を阻血にするため、大動脈基始部に鉗子が掛けられる。

 9) 心筋保護液の注入
  大動脈基始部または左右の冠動脈口より心筋保護液を注入する。これにより、心筋の活動は
 抑制され、心室細動であった心電図は平坦化する。

 10) 体外循環の維持
  術野において心内操作が開始されると、人工心肺側には大きな操作はしばらくなくなる。循
 環動態や温度に急激な変化はなくなるが、流量、血圧、尿量、血液検査、温度などのデータ監
 視がなされ、必要に応じ循環血液量、循環血流量、吹送ガス流量と濃度、温度などを適時調節
 する。

 11) 復温開始
  心内操作がほぼ終わると、術者と連絡し、復温を開始する。冷温水槽により熱交換器に温水
 を流して血液を温める。このとき送血温と脱血温の温度差は10℃以内とし、水温が42℃を超え
 ないように注意する。

 12) 大動脈遮断解除
  大動脈遮断解除に先立ち、手術操作により混入した気泡を除去するため、いったん心内に血
 液を充満させる。貯血槽には、これに対応する十分な血液が必要である。
  心内操作が終了すると、大動脈遮断鉗子が外される。この時点から冠動脈の血流は再開する。

 13) 心拍動再開
  冠動脈に血流が流れ込むと心筋保護液は流され、心筋は活動を始める。正規律動に戻らない
 場合は電気的除細動が行われる。
  心臓のポンプ機能の回復とともに、再び部分体外循環となる。

 14) 復温完了
  血液温の上昇は速いが、咽頭温、直腸温は上昇しにくいため、復温の良い指標となる。復温
 が十分でなく局所的にも温度が低いと、人工心肺離脱後しだいに低体温となり、最悪の場合、
 心停止の危険もある。

 15) 体外循環の終了
  止血、復温、心機能回復を確認後、体外循環から離脱させる。脱血量を徐々に減らすとともに、
 送血量も減らし体外循環を終了させる。
  離脱後しばらくは循環動態が安定しないため、緩やかな送血による循環血液量の調整を行い
 ながら心機能の安定を待つ。また、不慮の出血や、心機能の低下に伴い体循環を再開すること
 もある。

 16) カニューレの抜去
  止血、心機能回復を確認し、カニューレを抜去する。不慮の出血に備え、人工心肺の送血回
 路と患者の末梢ラインを接続する。

 17) プロタミンの投与
  抗凝固剤ヘパリンを中和するため、投与されたヘパリンとほぼ同量のプロタミンが投与され
 る。

 18) 人工心肺内残血処理
  残存血は、ゆっくり返血される。自己血回収装置や除水回路により、余分な水分を処理して
 から返血されることもある。

 19) 人工心肺の廃棄
  人工心肺回路はすべて廃棄するが、廃棄物取扱者の感染事故防止のため、十分注意して処理
 する。

Heart failure

 血液を全身に送り出すポンプとしての心臓の働きを調節する新しいたんぱく質を国立循環器病センター(大阪府吹田市)の北風政史・心臓血管内科部長らが発見し、米医学誌に発表した。

 心臓の働きが弱っている患者用に、副作用がほとんどない治療薬を開発できる可能性があるという。

 研究グループは、重症の心不全患者12人から同意を得たうえで心筋組織の一部を採取し、そこで働く遺伝子を調べた。その結果、筋肉を構成する主要なたんぱく質であるミオシンの一部に作用して筋収縮を起こすたんぱく質を見つけ、「心臓特異的ミオシンキナーゼ」と名付けた。

 拡張型心筋症の患者は、このたんぱく質の量が少なく、ラットなどの動物実験で、このたんぱく質を作る遺伝子の働きをおかしくすると、筋組織の構造が乱れ心臓が拡大した。心臓の形を整える役割も持つと考えられる。

 心不全患者の治療に現在使われている強心剤は、長期間投与すると、心筋の組織を傷める副作用がある。北風部長は「新しいたんぱく質は、心臓の働きの根幹を担っている可能性が高い。症状との関連を解明して、副作用のない治療に役立てたい」と話している。
